How does Bell Of Chaos work? Does it remove the item from all active decks and place those cards in their discard pile but when they reshuffle they are back in those decks or does it eliminate that item completely?

I'm almost positive it removes the card from all books, but once an exhausted book reshuffles, the removed card is returned. The only way to permenantly 'zap' a card out of existance for a game is Death Gaze.
